CPT 99211

From my understanding, 99211 is used many times for the wrong reasons. But the more research I do, the more I feel the code is allowed to be used even if the reason for the visit is for a different service such as blood work.

The document that I'm auditing at the moment states:

Reason for visit: Blood work.

CPT codes on the superbill: 99211, 36415.

On the progress note, it states: "lab work only" but it also includes the Blood Pressure. height, weight, and the BMI.

Would the Blood Pressure. height, weight, and the BMI be the reason for cpt code 99211?

Who drew the blood? Nurse, doc, lab tech? Were the vitals needed to evaluate the lab result?
I don't believe we are allowed to use both 99211 and 36415 (Collection of venous blood by venipuncture) unless a significantly separate service is performed in addition to the venipuncture. Also, your lab should be resulting the sample as well.
